Earlier menopause was linked to faster memory decline in an 18-year study

What happened

A study published in JAMA Network Open, and widely reported on 9 and 10 September 2026, has found that women who go through the menopause earlier tend to show faster decline in memory and thinking as they age, and are diagnosed with Alzheimer's disease slightly earlier. The researchers used data from 2,603 women taking part in two long running American ageing studies, the Religious Orders Study and the Rush Memory and Aging Project. None had dementia when they joined. They were followed for up to 18 years, with regular tests of memory and thinking, and in many cases brain scans. The pattern the researchers found was a gradient rather than a cliff edge. The earlier a woman's menopause, the faster her general thinking ability and her episodic memory, the memory for specific events in your life, tended to decline. The scale of the effect was modest. The models estimated that a woman whose menopause came ten years earlier than another woman's might have close to two fewer years free of Alzheimer's disease in later life. Brain scans pointed the same way. Women with an earlier menopause built up white matter hyperintensities, small areas of damage in the brain's wiring that show up on MRI, faster as they got older.

Why this matters

Around two thirds of people living with Alzheimer's disease are women. For decades that was mostly explained by women living longer. It is now clear that longevity is not the whole answer, and researchers have been looking hard at what else differs. The menopause is an obvious place to look. Oestrogen is active in the brain as well as the reproductive system, and the menopause is a substantial hormonal change that happens to every woman who lives long enough, at very different ages. In the UK the average age of menopause is around 51. Roughly 1 in 100 women go through it before 40, which is called premature ovarian insufficiency, and about 1 in 20 before 45, described as early menopause. For those women, this study is not a reason to panic, but it is a reason to be taken seriously. Early menopause is already known to raise the risk of osteoporosis and heart disease, which is why hormone replacement therapy is usually recommended for women with premature ovarian insufficiency at least until the usual age of menopause. Brain health belongs in that same conversation.

What the evidence actually says

This is an observational study. It followed people over time and looked for patterns. It cannot show that early menopause causes faster brain ageing. The researchers themselves were careful about this, and their caution deserves to travel as far as their headline. They pointed out that earlier reproductive ageing and earlier brain ageing may both be signs of one broader biological process, rather than one causing the other. Genes, general health, smoking and social circumstances all influence when the menopause arrives, and all influence brain health too. The study also has a specific limitation worth knowing. The participants were mostly older American women, including a large group of nuns, and were not fully representative of the general population. Findings from one group do not automatically transfer to another. And the size of the effect matters. Two fewer Alzheimer's free years across a ten year difference in menopause age is a real finding, but it is far smaller than the effect of the risk factors we already know how to change. The 2024 Lancet Commission on dementia estimated that around 45% of dementia cases worldwide are linked to 14 modifiable risk factors across a lifetime, including hearing loss, smoking, high blood pressure, physical inactivity, social isolation and air pollution. In other words: the age your menopause arrived is not something you chose or can change. Most of what shapes your brain health still is.

Practical advice

If you went through the menopause early, the useful response is a conversation, not a worry. Make sure it is recorded properly in your GP notes, with the age it happened. That single fact affects decisions about bone health, heart health and HRT for decades afterwards. If you had premature ovarian insufficiency, ask whether HRT is right for you. UK guidance generally recommends it, unless there is a specific reason not to, at least until around the usual age of menopause. This is a discussion to have with a clinician who knows your history, not a decision to take from an article. Focus your energy on the risk factors that are genuinely modifiable. Get your blood pressure checked, and treated if it is high. Get your hearing checked, and use aids if you need them, because untreated hearing loss is one of the larger contributors to dementia risk. Keep moving. Do not smoke. Keep up social contact, which is protective in a way that is easy to underestimate. And if you or your family have noticed memory changes that are getting worse, or that are affecting daily life, see a GP. Some causes of memory problems are treatable, including thyroid problems, vitamin B12 deficiency, depression and side effects from medicines.
